The American Health Planning Association conducts an annual survey of all 50 states and the District of Columbia and compiles the information in its National Directory * Certificate of Need Programs * Health Planning Agencies. The directory includes state specific summaries of key information including state contacts for health planning and policy, health data and Certificate of Need Programs. The directory also incorporates information on relevant legislation, data availability  and certificate of need program parameters.

The directory costs $125 and can be obtained by faxing/emailing a completed order form   to (directory@ahapnet.org) , by calling the American Health Planning Association (703-573-3103), or by faxing the completed form (703-573-3103). A brochure and sample pages offer additional information on the directory. All files are in Acrobat (PDF) format. Links to theses resources  are provided   below.
